99316 billed prior to 99306

A patient is admitted to a SNF on 04/08/2017. The NP sees the patient on 04/10/2017 and 04/12/2017 and bills the discharge on 04/12/2017, 99316. The admitting physician doesn’t see the patient until 04/13/2017 for the initial assessment. it is my understanding that the physician has 30 days from the date of admission to assess the patient and the NP is allowed to see the patient prior to the physician. My question is can I bill the discharge code on a day before the patient actually leaves the SNF? The discharge work was completed on one day and the patient is discharged on a subsequent day. If so, could I also bill the initial evaluation AFTER the discharge is billed?
