Ob/gyn coder/auditor for several years now.

I do have a question regarding during a ROB (routine OB)visit , Patient is requesting a toxo panel to be done due to her having indoor/outdoor cats. Patient is 36-6 weeks, with excessive weight gain of pregnancy, VSS and WNL.she reports doing well with no c/o today. +FM, no VB or LOF. Patient is desires breast pump packet today. Counseled on importance of continued tobacco cessation, patient counseled on contraceptions to in include LARC, Went over some clarified portions of patient history. All questions answered.
LMP: 20oct2017 EDC 27July2018. Obtrained samples for GBS screening. Transabdominal Ultrasound Obstetric – as a limited evaluation was performed 3.73 SDP, anterior placenta, FHR 140bpm, Cephalic Position. Lab results were given to patient and explained.
40minutes face to face floor time. follow up as needed in 1 week.
The provider is coding this with diagnosis: Excessive weight gain in pregnancy, third trimester (O26.03), BMI (
z68.31), Z3A.36 (weeks of gestations) and Z36.85 (screening GBS) along with 3294F (screening GBS). We use the 0502F for ROB visit, 76815 for TAVU-Limited. Then the provider coded the E/M level as a 99215.
My question is that I am not seeing why he should be using a E/M level of 99215. Can anyone else out there help me understand this?
Thank you,
Betty Hougasian, CPC
